While each treatment plan is individualized, clients in PHP typically participate in care several days per week during daytime hours. A typical week may include:
5-6 Days per week

Weekly individual therapy
Ongoing coordination with a Recovery Navigator
Trauma-informed care integrated across sessions
Medication management when clinically appropriate
Structured group therapy sessions focused on skill-building and recovery support
Treatment at Xolani does not end abruptly when PHP concludes. From the beginning, we plan for what comes next.
As clients stabilize, they transition smoothly into the appropriate next level of care, often Xolani’s Intensive Outpatient Program (IOP) or structured continuing support. Each client receives guided transitions, clear expectations, and ongoing coordination to reduce drop-off and protect progress.
For every client, the first 90 days after higher-level care include structured relapse-prevention support, ensuring accountability and continuity during the most vulnerable phase of recovery.
